Community Engagement
The goal of the Community Engagement Team is to achieve health equity in Howard County by identifying and reducing health disparities through collaborative partnerships and building trusting relationships with the community.
Services provided:
- Outreach and engage schools, health care settings, worksites, and community-based organizations to coordinate and deliver health education in the community.
- Develop, coordinate, implement, and evaluate evidence-based programs to empower community members to attain improved health outcomes and reduce health disparities in areas of highest need.
- Support the Local Health Improvement Coalition (LHIC) in assessing health needs and in setting and pursuing the key priorities, goals, and objectives of the Community Health Improvement Plan (CHIP).
- Increase awareness of and access to culturally appropriate, accessible, and inclusive Howard County resources and services through communication, special projects, CAREAPP, and collaborative partnerships supported by LHIC workgroups.
- Support and serve the Howard County Health Department to provide impactful and purposeful engagement through community led efforts and empowerment.
- Work collaboratively with community residents, Community Health Workers (CHWs), and organizations who are from, led by, or partner closely with the populations we seek to benefit.

Community Health Education Request Form
We are happy to provide educational materials, tabling, and in-person or virtual presentations to your organization. Our ability to honor your request is based on staffing and alignment with HCHD mission and vision. Please complete the Community Health Education Request Form at least one (1) month prior to the event date.
- If this form is completed one (1) month prior to the event, we will contact you to confirm or decline participation within five (5) business days.
- If this form is completed more than one (1) month in advance of the event, we will contact you to confirm or decline participation thirty (30) days before the event.
- If this form is completed less than one (1) month prior to the event, we may be able to offer HCHD program materials in lieu of HCHD staff participation.
Local Health Improvement Coalition (LHIC)
The Local Health Improvement Coalition (LHIC) works to achieve health equity in Howard County. Participation is sought from individuals and organizations working to achieve optimal health and wellness for all Howard County residents.
Healthy Howard County Campaign
The Healthy Howard campaign provides tips that you can use to improve your health. You can lower your chance of chronic health conditions (medical problems that last a long time) by eating healthy, exercising, getting regular check-ups, and living a nicotine-free lifestyle.
Community Health Workers
Community Health Workers (CHWs) encourage health and wellness by helping people in their communities engage in healthy behaviors. They support those who may have difficulty connecting to health resources and social services.
