School Based Wellness Centers
School-based clinics are located in 10 elementary, middle and high schools in the Howard County Public School System. They are made possible through a partnership between the Howard County Health Department and Howard County Public School System. The goal of the program is to keep kids in school, healthy and ready to learn.

Services Provided
School-Based Wellness Centers - In-person Visit
- Sick care
- Treatment of injuries
- Health education
- Physicals
- Well child physicals, sports physicals, and school entry physicals as well as immunizations for uninsured students or students with Medicaid.
- Immunizations for children without a health care provider
- Management of chronic health problems such as asthma and obesity with you and your child’s regular health provider.
- Behavioral Health Services. Services are not provided by the SBWC staff. We will refer you to outside behavioral health services as needed.
School-Based Wellness Centers - Telemedicine Visit (when on-site provider is not available)
- Sick care for problems involving the lungs, eyes, ears, nose, mouth, throat and exposed skin.
- Sprains and strains.
Parents are always welcome to join the visits from their phones, computers, or in person at the health suite.
Telemedicine visits are at NO CHARGE regardless of insurance status. If your child has health insurance and the insurer only pays part of the bill, you will NOT be responsible for any unpaid balance.*
The SBWC is NOT a primary care provider and will not replace your current care provider. The SBWC does not provide services outside of school hours. You should contact your primary care provider or urgent care provider during non-school hours.
*If you are a patient of Klebanow & Associates or Columbia Medical Practice and use the Program’s virtual platform, these visits are billed the same as office visits.

Eligibility
- Children must be enrolled in the schools that have SBWCs to receive services.
Cost
- The School-Based Wellness Centers offer NO COST health services for all students regardless of health insurance or family income.
- Program physicians and nurse practitioners provide health services only during the school day and do not replace students’ regular health providers.
- School-Based Wellness Centers will bill Medical Assistance only. There are NO copays or bills for uncovered charges.

Our Providers
Ambyr Klink RN, BSN, MSN, CPNP-PC - Program Supervisor

Ambyr is the SBWC Program Supervisor. She is the Wellness Center nurse practitioner at Phelps Luck Elementary School.
Ambyr has been a pediatric nurse practitioner since 2013. She received her BSN from Shepherd University and her MSN from University of Maryland, Baltimore. She worked at Johns Hopkins as a nurse and has had experience in a pediatric urgent care and primary care practice as a nurse practitioner before joining the School-Based Wellness team in 2022.
Ambyr graduated from Centennial High School. In her free time, she enjoys spending time with her family, reading, gardening, and spending time outside.
Annette Baldwin, MSN, APRN, CPNP-PC

Annette is the Wellness Center nurse practitioner at Bollman Bridge Elementary, Patuxent Valley Middle and Hanover Hills Elementary.
Annette is a Certified Pediatric Nurse Practitioner who earned her BSN from Bloomsburg University and her MSN from University of Maryland. She was a pediatric nurse at various children’s hospitals prior to beginning her work as a nurse practitioner at Baltimore City School health clinics in 1995. She joined the Howard County School-Based Wellness team in 2014, and loves providing care for young people at her schools.
Annette and her husband Rob live in Ellicott City and have five children who attended Howard County Public Schools. Pickleball and family game night lead the list of her favorite activities. She always enjoys spending time with family and friends.
Alex Dmuchowski, MSN, BSN, APRN, CPNP-PC

Alex is the Wellness Center nurse practitioner at Talbott Springs Elementary, Cradlerock Elementary and Lake Elkhorn Middle.
Alex has been a Certified Pediatric Nurse Practitioner since 2016. She received her BSN from Johns Hopkins University and her MSN from The Catholic University of America. Her past employment experience includes pediatric medical/surgical/burn nursing, adolescent medicine, and pediatric primary care.
Alex is a Howard County native and a graduate of Mount Hebron High School. She enjoys reading, traveling, and exploring local events with her daughter and her dog. She joined the School Based Wellness Program in 2023 and is looking forward to serving Howard County students!
Laura Phebus, CNA, CMT, EMT-B

Laura visits all of the Wellness Centers.
Laura has been a Certified Nursing Assistant (CNA) since 1996. She received her Bachelor’s degree at Kalpan University in 2015. She has worked in several hospitals, urgent cares, and Howard County Public schools before joining the School-Based Wellness team in 2023.
Laura graduated from Mount Hebron High School. In her free time, she loves working in garden, playing bingo, singing karaoke, her dog “Frankie” and being with friends and family.
Stephanie Soukup, MD
Stephanie is a board certified pediatrician. She received her medical degree from the University of Maryland School of Medicine in 2012 and completed pediatric residence and chief residency at Sinai Hospital of Baltimore. She practiced general pediatrics in Olney, MD before taking a full time position outside of clinical practice in 2018 when she also joined the School-Based Wellness Program as a volunteer.
Stephanie is a Howard County native and graduated from Glenelg High School. She enjoys spending time with her husband and two children, gardening, and cooking.
